Prior to contacting the Parish Office there are steps that should be taken in preparation:
- If you, as a parent or guardian, are not already a registered member of the St. Francis Xavier Church, you must complete that process before having your child baptized. You can register on-line HERE through My Own Church or contact the parish office (502-538-4933) for assistance.
- Complete the Registration Form and submit to the Parish Office in person, through the mail, or by emailing to paula.silliman@sfxmw.com. Download the form HERE. (on-line forms are currently under construction).
- Download and complete the Photo Release Form to allow pictures to be posted to the SFX website, or complete on-line HERE.
- Watch the required six (6) REBORN videos and complete the questions associated with each session.
- Access REBORN videos through FORMED. If you are not taken directly to the Reborn section of FORMED, select Programs…The Sacraments…Reborn: You, Your Child, and the Heart of Baptism.
- Download the questions that accompany the 6 videos.
- Contact the Parish Office at (502) 538-4933 to schedule an appointment with Paula Silliman to hand in paperwork and schedule the sacrament. If Godparents are known at this time, download the Godparent Form HERE and bring to the appointment. Godparent forms must be submitted prior to the date of the sacrament.
